Ladders are one of the most common ways employees and contractors access elevated work areas—and one of the easiest pieces of equipment for a facility to overlook.
A ladder may have been installed decades ago. Mechanical equipment may have been added around it. A roof hatch may have been replaced. A new parapet may have changed the transition onto the roof. Or a fixed ladder may simply have remained in service long after OSHA’s walking-working surface requirements changed.
For commercial building owners, facility managers, safety professionals and contractors, OSHA 1910.23 is the primary general-industry standard governing ladders.
The rule addresses portable ladders, fixed ladders and mobile ladder stands and platforms. It establishes requirements covering ladder dimensions, condition, use, inspection and access.
But OSHA 1910.23 isn’t the entire story.
For fixed ladders, OSHA specifically points employers to 1910.28 for the duty to provide fall protection and 1910.29 for the criteria that those systems must meet.

What Does OSHA 1910.23 Cover?

At least 10 inches and no more than 14 inches apart, measured between their centerlines.

OSHA also requires rungs, steps and cleats to be parallel, level and uniformly spaced when the ladder is positioned for use.

- 11.5 inches for portable ladders

A ladder that originally had adequate climbing clearance can become compromised as a rooftop or mechanical space evolves.

Ladders must be inspected before initial use in each work shift, and more frequently when necessary, for visible defects that could cause injury.

A practical ladder inspection should consider issues such as:
- Damaged or missing rungs

Safe access is more than simply having a ladder available.
OSHA Portable Ladder Requirements
Portable ladders introduce additional hazards because their location and setup change with each use.
OSHA requires portable ladders to be used on stable and level surfaces unless they are secured or stabilized against accidental displacement. Portable ladders used on slippery surfaces must also be secured and stabilized.
Ladders located in areas such as passageways, doorways or driveways where traffic could displace them must either be secured or protected by a temporary barricade.
OSHA also prohibits several common practices:
- Moving or extending a ladder while someone is on it
- Using portable single-rail ladders
- Standing on the cap or top step of a stepladder
- Fastening ladder sections together unless designed for that purpose
- Placing ladders on boxes, barrels or other unstable bases to gain additional height
These requirements are particularly relevant during temporary commercial maintenance projects.
The OSHA 3-Foot Ladder Extension Rule
One of the most commonly searched OSHA ladder requirements involves accessing an upper landing.
When a portable ladder is used to access an upper landing surface, OSHA requires the ladder’s side rails to extend at least:
3 feet above the upper landing surface.
This provides workers with a more secure transition when moving between the ladder and the elevated surface.
Transitions are important because the worker must change from climbing vertically to stepping horizontally onto a roof or platform.
OSHA Fixed Ladder Requirements
Fixed ladders are especially important for commercial building owners because they frequently provide permanent access to:
- Rooftops
- Mechanical penthouses
- Equipment platforms
- Cooling towers
- Tanks
- Mezzanines
- Elevated machinery
- Maintenance areas
OSHA requires fixed ladders to support their maximum intended load.

7 inches
from the centerline of the ladder’s rungs, steps or grab bars to the nearest permanent object behind the ladder.
Elevator pit ladders have a separate minimum of 4.5 inches.

Fixed Ladder Side-Rail Extension
For through or side-step fixed ladders, OSHA requires the side rails to extend at least:
42 inches above the top of the access level or landing platform.
Parapet ladders require additional consideration because OSHA defines the access level differently depending on whether the parapet is continuous or has been cut to permit passage.
These transitions are one reason rooftop ladder assessments should evaluate more than the ladder itself.
Fixed Ladder Step-Across Distance
OSHA establishes different transition dimensions for through ladders and side-step ladders.
For through ladders, the step-across distance from the centerline of the rungs or steps generally must be:
7 to 12 inches to the nearest edge of the structure, building or equipment being accessed.
For side-step ladders, OSHA requires:
15 to 20 inches to the access point at the platform edge.
Poor transitions at the top of a ladder can create hazards even when the ladder itself is otherwise structurally sound.
Fixed Ladders and Roof Hatches
Roof hatches are another common commercial access point.
When a fixed ladder terminates at a hatch, OSHA requires the hatch cover to open with enough clearance for easy access to or from the ladder.
If the hatch is counterbalanced, it must open at least:
70 degrees from horizontal.
Facility managers should evaluate the entire transition:
ladder → hatch → rooftop landing area → fall hazard.
The presence of a compliant ladder does not automatically make the rooftop access point safe.
When Do Fixed Ladders Require Fall Protection?
This is one of the most important distinctions in the OSHA ladder standards.
OSHA 1910.23 establishes the ladder requirements, but 1910.28 establishes the employer’s duty to provide fall protection for employees on fixed ladders, while 1910.29 establishes criteria for those systems.
Under OSHA 1910.28, fixed ladders extending more than 24 feet above a lower level are subject to specific fall-protection requirements.
The exact requirements depend in part on when the ladder was installed and whether it has been replaced or modified.
This distinction matters because older facilities may still have ladder cages or wells that were accepted under the transition provisions but are not the solution OSHA requires for newly installed fixed ladders.
Facility managers should therefore avoid assuming:
“It has a cage, so we’re compliant.”
The ladder’s installation date, height, configuration and fall-protection system all need to be considered.
OSHA 1910.28 fall-protection requirements
Ladder Cages vs. Ladder Safety Systems
This is particularly important for commercial properties with older fixed ladders.
Under OSHA’s updated walking-working surfaces rules, ladder cages are being phased out as the primary fall-protection method for fixed ladders extending more than 24 feet.
The requirements generally work on a timeline:
New fixed ladders installed on or after November 19, 2018: must be equipped with a personal fall arrest system or ladder safety system.
Existing fixed ladders extending more than 24 feet: OSHA established transition provisions for cages, wells, personal fall arrest systems and ladder safety systems.
By November 18, 2036: fixed ladders extending more than 24 feet must be equipped with a personal fall arrest system or ladder safety system.
That 2036 deadline sounds distant, but for building owners managing dozens or hundreds of fixed ladders, replacement planning should happen well before the deadline.
Why Rooftop Ladder Safety Requires More Than a Ladder Inspection
Consider a typical commercial rooftop.
A technician:
- Enters a mechanical room.
- Climbs a fixed ladder.
- Opens a roof hatch.
- Steps onto the roof.
- Walks toward an HVAC unit.
- Performs maintenance near an unprotected roof edge.
The ladder might comply with 1910.23.
But that doesn’t necessarily mean the work path complies with OSHA’s broader fall-protection requirements.
Under 1910.28, employees on walking-working surfaces with unprotected sides or edges generally must be protected when they are 4 feet or more above a lower level, subject to the standard’s exceptions and specific provisions.
That’s why commercial rooftop safety should be assessed as an interconnected system.
Common Fixed Ladder Problems in Commercial Buildings
In the field, facility teams should pay particular attention to situations such as:
Mechanical equipment added around a ladder
New piping, ductwork or equipment can reduce OSHA-required clearance.
Poor roof transitions
Workers may reach the top of a compliant ladder only to encounter an exposed edge or awkward step onto the roof.

A cage can create the impression that the ladder’s fall-protection requirements have been permanently addressed even though OSHA’s transition requirements need to be considered.
